Instructions

1. Sharer to complete Section 1 & 2

2. Borrower to complete Section 3 upon pickup. Please note - Borrower must confirm he/she is familiar with the safe operation of the gear being borrowed. A photo of the borrowers driving license will need to be uploaded at this time.

3. Both Borrower and Sharer to complete and sign section 4. After this, the form will be emailed to the Sharer ready to be signed and submitted when the borrow is complete.

4. Both Sharer and Borrower to complete and sign section 5 at the conclusion of the borrow.
