LVL Lash Lift consent form
The purpose of this informed consent form is to provide written information regarding the risks, benefits and alternatives of the procedure named above. This material serves as a supplement to the discussion you have with your practitioner. It is important that you fully understand this information, so please read this document thoroughly. If you have any questions regarding the procedure, ask your practitioner prior to signing the consent form.
THE TREATMENT
LVL Lash Lift enhances your lashes without the use of extensions and gives your natural lashes a beautiful curl lasting 6 – 8 weeks. The treatment takes just 55 minutes and it is a low maintenance, natural-looking treatment with minimal aftercare! It works by straightening the lashes at the root giving them the appearance of “Length, Volume and Lift”. There is no need for any maintenance appointments in between but you will need to have the process repeated every 6 – 8 weeks to lift the new hair growth. To keep your lashes looking blacker than black, return to the salon after 4 weeks to top up your tint.
The Process
First, your lash artist will apply the appropriate size shield to your eyelid and your lashes will be pressed against this using a bonding gel.
With all the lashes neatly bonded, a lifting balm is then applied, which allows the structure and shape of your lashes to change and be lifted at the root.
Step 2 – Volumize
Once this has been removed, a volumising fix is applied, which seals your lashes into their new shape.
Step 3 – Tint
An eyelash tint is then applied which darkens the lashes.
Step 4 – Moisturise
Finally, a moisturising serum is applied to nourish the lashes and ease them off the shield, revealing beautifully healthy, lifted lashes.
RISKS AND COMPLICATIONS
I have been advised that the goal of the procedure I have requested is improvement in the appearance, not perfection, that there is a possibility that imperfections might ensue, and that the results may not meet my expectations or the goals that have been established. In relations to this I know that the practice of beauty is not an exact science and that, therefore, no guarantee or assurance has been made by anyone regarding the procedure, which I have herein requested, and autorised. Although good results are expected, there is no guarantee or warranty expressed or implied on the results that may be obtained. Before undergoing this procedure, understanding the risks is essential. No procedure is completely risk-free. The following risks may occur, but there may be unforeseen risks and risks that are not included on this list. It has been explained to me that there are certain inherent and potential risks and side effects in any invasive procedure and in this specific instance such risks include but are not limited to: • Pain – It can be uncomfortable at the time of treatment and for a variable time after – painkillers can be taken in the first 24 hours to relieve discomfort. • Swelling and Redness – This is a normal response and usually settles after 2-3 days. The treated area may also ooze a little. • Puffiness • Blistering • Infection – This is very uncommon, and results in increased pain and formation of pus. Antiseptic or antibiotic therapy may be required. • Pigment changes. Both hypo pigmentation (lightening of the skin) and hyper pigmentation (darkening of the skin). Both generally last a few months but can be longer. I understand that sun exposure, fake tan or sun beds are not advised during my treatment programme as this may result in pigment changes or markings on my skin that may not fade. Use of sunblock or SPF 50+ is recommended. I have been advised that hot baths/ showers or any other heat treatments should be avoided for a minimum of 24 hours. I have been advised to keep the area clean and dry and to always allow scabs to heal naturally.
Aftercare
Lash lifts take approximately 24 hours to fully set in place, so for the first 24 hours:
- Do not get your lashes wet.
- Avoid contact with steam (sauna, facial steamer etc.).
- Do not wear mascara.
- Be gentle with your lashes; no rubbing.
Additional aftercare tips
- Avoid waterproof mascara as removal can be harsh and therefore weaken the lift.
- Use oil-free eye make-up remover to make the tint last longer (however, this has no effect on the lift).
- Regular use of a lash conditioning serum (e.g. LVL Lash Lift Serum, which can be purchased at our salon) is recommended to keep your lashes well nourished.
FAQ
Are there different levels of lift to choose from?
There are three different shield sizes used to create different levels of lift: Small shield (most dramatic lift), Medium shield (medium lift), Large shield (most natural lift). If you find you want a more dramatic or more natural lift next time you can ask your therapist to alter the shield used on your next visit.
Does LVL Lash Lift damage your natural lashes?
No, an LVL Lash Lift does not damage your natural lashes when carried out by a qualified specialist. Our clients have constantly had lash lifts every 6 – 8 weeks and they have never encountered their lashes being damaged. To keep your lashes well-nourished, regular use of a lash conditioning serum (e.g. LVL Lash Lift Serum, which can be purchased at our salon) is recommended as a part of your aftercare.
Can you still wear mascara with an LVL Lash Lift?
Although an LVL Lash Lift includes an eyelash tint which helps to create the appearance of longer and thicker lashes without the need for mascara, it’s still completely safe to wear should you wish to enhance the results further. However, we recommend waiting until 24 hours after your treatment.
Does LVL Lash Lift work on everyone?
An LVL Lash Lift works on nearly all clients, however as this treatment only works with the natural lashes, the longer your lashes, the better the results will be. If you have very short natural lashes then extensions may be a better option for you.
Does getting an LVL Lash Lift done hurt?
No, getting an LVL Lash Lift done is not painful – in fact, most clients find themselves drifting off to sleep! If at any point you feel any irritation, tell your therapist straight away.
Can an LVL Lash Lift be reversed?
Yes, an LVL Lash Lift can be reversed or the curl can be reduced by relaxing your lashes, which is a quick procedure that only takes around fives minutes.
RIGHT TO DISCONTINUE TREATMENT
I understand that I have the right to discontinue treatment at any time.
Photographs and videos will be used for scientific and marketing purposes both in publications and presentations. Photographs and video may be taken of me for educational and marketing purposes. I hold the clinic harmless for any liability resulting from this production. I waive my rights to any royalties, fees and to inspect the finished production as well as advertising materials in conjunction with these photographs.
RESULTS
Most patients are pleased with the results of the treatment use. However, like any aesthetic procedure, there is no guarantee that you will be completely satisfied. There is no guarantee that you will not require additional treatment to achieve the results you seek. I am aware that follow-up treatments will be needed to maintain the full effects. I am aware the duration of treatment is dependent on many factors including but not limited to: age, sex, tissue conditions, my general health and life style conditions, and sun exposure. I have been instructed in and understand the post-treatment instructions. I understand this is an elective procedure and I hereby voluntarily consent to treatment. The procedure has been fully explained to me. I also understand that any treatment performed is between me and the doctor/healthcare provider who is treating me and I will direct all post-procedure questions or concerns to the treating clinician. I have read the above and understand it. My questions have been answered satisfactorily. I accept the risks and complications of the procedure and I understand that no guarantees are implied as to the outcome of the procedure. I also certify that if I have any changes in my medical history I will notify the doctor/healthcare professional who treated me immediately. I also state that I read and write in English.
PAYMENT
The procedure cost includes the professional fee for the injections, follow up visits to monitor the effectiveness of the treatment, and the cost of the material itself. Additional costs of medical treatment would be your responsibility should complications develop from injections.
I hereby authorise my practitioner to administer the treatment and agree to hold him free and harmless from any claims, refunds or suits damages for any injury or complications whatsoever which may result in the treatment.
I understand that my treatments require payment at the time of service and the prices and fee structure for treatments have been explained to me.
There are no refunds on treatments or on treatments paid for in advance.
